Care Integration Service (Community Care)
Provides approval for Care in the Community for services not available within the VA, or for services with long wait times. Care includes routine outpatient services, including specialty care, Adult Day Health Care, Home Maker/Home Health Aid, Palliative and Hospice Care, Veteran Directed Home Care, Respite Services, and Community Nursing Home. Care in the Community does require certain eligibility requirements to be met.

Adult Day Care
In Home Care
In Home Care consists of programs that manage the veterans care at home, allowing for independence and sustained function at home. Services include —
- Skilled and Unskilled Home Care
- Physical Therapy
- Occupational Therapy
- Veteran Directed Care
- Home Respite
- Home Health Aide/Homemaker
- Home IV therapy
Inpatient Care
Inpatient care needs can also be addressed through Community Care. Services include —
- Contract/Community Nursing Home
- Palliative Inpatient
- Respite Inpatient
- Hospice Inpatient
- Surgery and post-surgery care
