Aleda E. Lutz VA Medical Center, Saginaw, Michigan
The Aleda E. Lutz Homeless Programs provide substantial hands-on assistance, coordinating with both VA and Non-VA Organizations to assist Homeless Veterans and their families to secure permanent housing. The Aleda E. Lutz Homeless Programs include Health Care for Homeless Veterans (HCHV), Grant and Per Diem (GPD), and HUD Veterans Affairs Supportive Housing (VASH). The HCHV Program is an entry point for Homeless Veterans to be connected with services and case management. The HCHV Program collaborates with 6 shelters in the catchment area to provide shelter beds (36 shelter beds) to Homeless Veterans until more permanent housing can be secured. The GPD Program provides transitional housing for up to 24 months; the Aleda E. Lutz VAMC has 1 location to serve our catchment area, with a total of 24 beds. The HUD VASH program is a partnership with HUD and VA to provide Homeless Veterans and their families with rental subsidy (Section 8) towards permanent housing and case management to assist Veterans to become connected to services and resources to prevent future homelessness and improve quality of life.
In addition, the Aleda E. Lutz, Homeless staff also works closely with community providers to assist Veterans to access resources to assist them towards permanent housing. One of the programs offered by community providers is Supportive Services for Veteran Families (SSVF), a grant funded program that can provide funds to assist Veterans with security deposit, outstanding utilities and offers the Rapid Rehousing Program which can provide financial assistance towards rent for a limited amount of time, often used to assist Veterans to obtain permanent housing while in the process of completing HUD VASH enrollment to prevent homelessness during interim towards permanent housing or for Veterans that do not qualify (i.e. over-income qualifications) for HUD VASH.
The Aleda E. Lutz, Homeless Programs provide outreach, clinical assessments, education about VA programs and community resources, referrals, rehabilitation and case management services. Homeless services are offered throughout the service area including Homeless staff designated to the CBOC’s at Cadillac, Traverse City, Alpena, Cheboygan, Bad Axe and Gaylord. The team is dedicated to ending homelessness among Veterans.
